Our Team

The Desierto Home Healthcare team works together to insure the best quality homecare for all of our patients. All of Desierto Home Healthcare professionals meet our rigorous employee requirements and have graduated from an accredited school for their specialization. Additionally, each skilled employee receives extensive orientation with Desierto Home Healthcare’s operational policies and procedures. All disciplines must maintain their continuing education hours each year and attend company in-services to increase their knowledge base and to keep current in their respective fields. Finally, each discipline adheres to a code of ethics and accountability.

Administrator

  • Responsible for planning, coordination, management and direction of all activities and programs of the agency.

Quality Improvement Coordinator

  • Oversees all aspects of quality control within the agency. Works with the Case Manager to identify areas of improvement for the agency staff.

Medical Director

  • Provides medical consultation to all agency personnel. Provides presentation training and orientation sessions, and community forums, as needed.

Director of Clinical Services

  • Responsible for the overall direction of home health clinical services. Establishes, implements and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.

RN Case Manager

  • The Case Manager is responsible for care coordination and case management. The Case Manager assures that care and services are delivered to meet patient needs and are according to physician orders.

Registered Nurse (RN)

  • The RN is responsible for the implementation of standards of care for skilled services as prescribed by the physician and in compliance with applicable laws and regulations and policies of the agency.

  • Completes the Initial Assessment Visit and coordinates the service the patient receives from admission/referral to discharge.

Physical Therapists

  • Physical therapists provide evaluation, treatment, and instruction to increase range of motion, strength, and endurance.

Occupational Therapists

  • Occupational therapists provide activities and instructions designed to increase independence for the patient unable to perform activities of daily living.

Speech Therapists

  • Speech therapists provide services designed to help patients improve or restore communication skills.

Medical Social Workers (MSW)

  • Medical social workers provide social, environmental, and emotional counseling for patients.
